Working to improve the health of people with diabetes by providing and facilitating Diabetes Self-Management Education (DSME) training classes through partnerships with providers and community organizations throughout our network. DEEP™ is a diabetes self-management program that has been shown to be successful in helping participants take control of their disease and reduce the risk of complications. DEEP™ was developed by the University of Illinois at Chicago for use in low-income, racial, and ethnic minority populations. It includes a training-of-trainers curriculum designed to engage community residents.

An example of this service is the Diabetic Empowerment Education Program, or “DEEP.”: In partnership with Council on Agings, Faith-based organizations, public libraries, physician offices, and senior living centers, we trained thousands of Medicare beneficiaries and their caregivers to self-manage their diabetes for better health outcomes. We trained 70+ community peers to sustain this project in Louisiana. AdviseWell’s SMP grants have educated over 305,000 seniors, healthcare providers and law enforcement to Protect, Detect and Report healthcare fraud abuse and waste.
