Community Coalition: defined as “a group of individuals representing diverse organizations, factions, or constituencies within the community who agree to work together to achieve a common goal” (Feighery & Rogers, 1990). Community coalitions can improve the relationship between the individual members and/or organizations, who had never before had the opportunity to collaborate. Apart from fulfilling the short and long-term goals, such collaborations also strengthen the community as a whole. Community coalitions are typically created when there is a need for a shared response to a community concern.

eQHealth AdviseWell has relationships with hospitals, nursing homes, home health agencies, hospice agencies, public health agencies, End Stage Renal Disease (ESRD) Networks 8 and 13, professional organizations, patients, physicians, clinicians, and others. eQHealth AdviseWell, therefore, is strategically positioned to work in partnership with community organizations, health care providers, patients, and families, to build learning communities and conduct quality improvement initiatives. eQHealth AdviseWell has worked throughout Louisiana to establish 6 Community Coalitions during Medicare QIO work focused on reduction of readmission to emergency room and acute care.
